Diamond Geriatrics staff are professionals in health care. We understand how the systems work and how to find who to talk to in order to advocate on your behalf to help you get the services and answers you need.
More important, we know what your rights are. We can help you ask the questions about your care and health that you may not have known to ask, or did not feel comfortable in asking. We can help families get the best care for their loved ones by being at care conferences, going to the doctor, following them while they are in hospital, being actively in the discharge planning process.
Health Care Navigating
Many times people have said to us, you are really like “Health Care Navigators.” That fits. Our role is often a combination of the advocate, the planner, and the guide, helping you find the best, simplest, and most effective way through the systems and to the people you need to be in touch with.
We can be by your side during this process or we can coach you as you go through it. And like a good Navigator, our goal is to get you through safely, all the way.
